Use this form for submitting digital images of insects or arthropods.

Instructions: Fill in the form below with the requested information. A * next to a field denotes required information. Use "NA" if you have no information for the required field. The more information you supply regarding the submitted images and problem, the better we will be able to assist you in identification/diagnosis of the problem.

To upload image file(s), click the "Choose file" or "Browse" button(s) in the Digital Image section and use the open window to find where the image is located on your computer. Select the image you want to upload and then press the "Open" button. The image file name will then appear in the "Image to Upload" field.

If you made a mistake, you may click the "Clear" button to clear the form and start over.

When you have selected all the images that you wish to upload, click the "Submit" button. Please be patient, your image(s) may take a while to upload.

If you are also submitting a physical sample to the lab, please check "Yes" where requested on the submission form.

When the image(s) have been transferred, a summary page with the sample information will be displayed and a confirmation email message will be sent to the Submitter's email address.
